Any fracture patients MUST be scheduled by calling the office directly.
 
Thank you for choosing Dr. Chetham for your medical needs. Please be sure to read all of the following information.

Please note that Dr. Kirelik treats patients between the ages of kindergarten - high schoolers for 
CONCUSSIONS ONLY. Concussion appointments can be 2 – 2 ½ hours long. When scheduling an appointment with Dr. Kirelik, please be sure to include the date of when concussion happened and how it occurred. Our Concussion department is an ACUTE concussion clinic, meaning we manage patients that have had a concussion within the 3 – 6 months of their injury. We do not see patients that are more than 6 months from their injury that caused the concussion. Please contact your primary care provider, if it has been more than 6 months. 

If you need baseline testing, please call the office directly at (720) 979-0840. If the patient is new and over 18, please contact Blue Sky Neurology.

If you cannot find an appointment time that works for you, please call the office directly at (720) 979-0840 and we’ll be happy to accommodate you.

New patients can download and print the New Patient Paperwork by clicking here. Please be sure to bring the completed forms with you and arrive 30 minutes prior to your scheduled appointment. Please also be sure to bring any x-rays, MRIs, ultrasounds, or CT scans to the appointment as well. A parent or legal guardian must be present for the appointment. 

All patients, please bring the following to expedite your registration process:
*A list of medications and dose
*Insurance card(s)
*I.D. or Driver’s License 
*The patient's social security number
*Any applicable co-pay


Please note that we cannot accept the following insurance plans: 
*Denver Health Medicaid (without a referral)
*Kaiser (without a referral)
*Humana
*Anthem CU
*Cigna CU
*KanCare
*Centura Exchange Plan
*United Healthcare All Savers Insurance

We require a referral for patients with the following insurance plans prior to being scheduled:
*Cigna Connect
*Kaiser
*UHC Navigate/United Healthcare One/ United Healthcare Compass (United Healthcare HMO)
*Aetna HMO
*TriCare Prime
*Denver Health Medicaid


Unless your x-rays were taken at the following facilities, please be sure to bring the disc with you to your appointment:
*Centennial Medical Plaza
*The Medical Center of Aurora
*North Suburban Medical Center
*Presbyterian/St. Luke’s Medical Center
*Rose Medical Center
*Sky Ridge Medical Center
*Swedish Medical Center
